Are they possible? Either out of the box, or in a nice cross-platform way? Thanks

MaxGui does support them, so they should work cross platform, but I haven used them on Macs yet. You need to have two icons or the same icon twice in the toolbar iconstrip and the toggle flag on. The first icon is shown as normal and the second when in selected state.
