Is it possible to create a B3D file format, with a mesh who have some child mesh , and keep this information ?

It is of course possible (otherwise there wouldn't be any animated models stored in b3d format.) I'm assuming your question is something specific but you didn't bother to explain. Please explain your question more specifically.
