Does BB3D work on macs?

No, there is a BMAX Version for Mac, see "Products".

Yes, if you want to run b3d programs on a Mac, use BlitzMax + MiniB3D.
MiniB3D is free.

ok, this is redundant but I have to say it...

What are the downsides to MiniB3D in comparison to B3D?

Yes, and Linux as well. Minib3d could have some issues regaring parenting and turning. I didn't get EntityAlpha to work on my PC. That could be related to minib3d using OpenGL instead of DirectX.
A nice feature about minib3d is that you can read and edit the source. So if you run into any problems, theoretically, you can solve them yourself.

MiniB3D uses OpenGL, needs BlitzMax and is crossplatform
B3D uses DX7 and is restricted to Windows.
